While on vacation in California on a very sunny day I was shooting with a sling mounted to the tripod socket the hood must have popped off while I was walking. One of the release buttons must have hit my hip and off it went without me knowing until I tried to snap a picture in bright sunlight. I've been using a sling for five years with my Olympus cameras with 12 different lenses. I have never had a lens hood pop off until the LH-66 decided to depart on its own.
Serious design flaw! I'm looking for alternatives but what I realy want is for Olympus to design a lens hood with a positive lock for the m.Zuiko 12-40 mm Pro. It is well past a reasonable amount of time to fix this problem based on all the complaints about this problem on various blogs and reviews.

A good lens hood
I use this hood quite often and unlike the others. I have not have the hood fall off yet.
I can understand how it could be an issue but so far I have lucky. The hood is made of some kind of hard plastic, so if it does fall It will most likely break. Otherwise, as a lens hood it does work without it vignetting the images throughout the zoom range. The neat thing about this hood is that it can be installed backwards and still lock onto the lens for storage

Yes it falls off. Mine split into two pieces that interlocked but do no more. One of the buttons came unglued. I agree that for a 1000 lens, better quality is needed. I will see if they will replace it! As a result there so a piece of wire sticking out that frequently pierces me. I was on a trip when it broke, so I continued to use the lens.

Add me to the list of those who find it unacceptable that a $45 hood for a $1,000 lens falls off spontaneously.
One of the reasons I sold my E-M5 Mk. I and went for a year with another mirrorless (APS-C) was the eyecup falling off the E-M5. I was swayed back to Olympus because the technical rep told me that problem had been fixed (along with the squinty and fatiguing viewfinder of the original). But now I have something else that falls off an expensive camera supposedly suited to professional use.
Look, Olympus: I demand reliability from my equipment, and "bits fall off" doesn't meet that criterion. Maybe I'm overly sensitive, having spent most of my adult life in both professional and recreational activities where reliable equipment literally means the difference between life and death. You cram a lot of innovative tech into your cameras, but everything after Aperture, Shutter, and ISO is gravy to me. I like m4/3, and I love your glass. Let's make it all reliable, shall we?

I've found this hood to be excellent. The build quality is great - with small buttons used to release the lock and a beautiful machined aluminium ring at the base of the hood. I've paid a lot more than this for Canon hoods in the past that haven't been as nicely made.
I have had no issues with the hood falling off unexpectedly.

It works fine when it's on but it falls off way too easily. If there were an alternative I'd use it. On my second one I intend to use a lens cap holder. There are descriptions on the Internet of beefing up the spring.
